Day Program
Welcome to our Day Hospice Program
​Under the leadership of a Registered Nurse Coordinator and a dedicated team of highly trained, compassionate volunteers, the Day Hospice Program provides clients with a day away from their usual medical routines. In a home-like setting, clients can connect with others, participate in social activities, and enjoy a supportive space outside the home. Caregivers also benefit from the program by having time to rest and focus on self-care, knowing their loved one is well supported. All services are provided at no cost, and transportation may be available when possible.
The Day Hospice Program runs every Tuesday from 10:00am-2:00pm.
Eligibility
The Day Hospice Program is designed for individuals living with a progressive, life-limiting or life-threatening illness. Because Day Hospice operates outside of the home, clients need to be able to manage their own personal care and medications and be able to transfer in and out of a vehicle with minimal assistance.

​The Hospice Hub programs and services are available to those who use the Perth and Smiths Falls District Hospital. When in doubt, reach out. We can refer you to your local hospice if we are not it.
