The Hospice Hub
The Hospice Hub is a community-led initiative that provides comprehensive, compassionate, and integrated services to support anyone at end of life and bereavement, including their family and caregivers wherever they call home. We provide education, information, system navigation, and responsive compassionate support services to allow people to live well with as much comfort as possible.

We have made it our mission to ensure that our community has access to hospice care wherever they call home.

We envision a compassionate community where everyone can live, age, die and grieve well.

Our hospice care focuses on the care, comfort, and quality of life of a person with a serious illness that is approaching the end of life.

We offer emotional, spiritual, navigation and bereavement support for both clients and their loved ones.
