Hospice Visiting
Our Hospice Visiting Program offers emotional support and practical help to clients facing a life limiting illness, wherever they call home. The Hospice Hub team provides support to trained volunteers who make regular, sometimes weekly, home visits. Hospice volunteers provide someone to listen, to read, and to talk with. Volunteers can assist at mealtime or simply watch over while the client rests. This program allows caregivers to do errands or to rest, knowing that their loved one is being cared for.
Day Program
The Day Hospice Program offers individuals living with a progressive, life-limiting, or life-threatening illness a welcoming, home-like environment where they can enjoy social activities, connect with others, and take a break from their usual medical routines. Led by a Registered Nurse Coordinator and compassionate volunteers, the program runs every Tuesday from 10:00 am to 2:00 pm at no cost, with transportation available when possible. Participants must be able to manage their own personal care and medications and transfer in and out of a vehicle with minimal assistance.
Grief & Bereavement Support
Support for the family and caregiver during an illness and after the death of a loved one. The Bereavement Program is led by a group of dedicated volunteers who help clients find comfort and peace even during grief. Through contact or referral to the Bereavement Coordinator, bereaved who have experienced a death can participate in our bereavement group programs.
Education
The Hospice Hub offers no cost, free to participants Advance Care Planning sessions throughout the area we service. Days and locations may vary so please visit our website for upcoming ACP sessions or contact our office to inquire about booking one with your venue or organization of choice.
Caregiver Support
We are pleased to announce the launch of Practical Caregiver Training, a new educational program. PCT offers structured, hands-on instruction designed to equip caregivers with essential skills for providing safe, compassionate, and effective care at home. This program provides significant benefits to caregivers, including increased confidence in performing daily care tasks, reduced stress through improved understanding of what to expect, and strengthened communication with hospice professionals.
Hospice Residence
The Hospice Hub is working towards establishing a Hospice Residence in our community. In 2018 this region was allocated four hospice palliative care beds. The Hospice Hub is working towards actualizing these beds.
